HOW TO APPLY FOR CITIZENSHIP ON YOUR OWN



How it Works

Citizenshipworks is a free  service that helps you apply for citizenship online on your own. This service will guide you through the process of your eligibility and citizenship application and then connect you to our initiative’s immigration provider organizations for an easy, quality, free final review. For instructions on how to use the portal see below.

How to Use Citizenshipworks – Step by Step Instructions

In order to ensure efficiency and timeliness, we ask that you begin your application online through a website managed by our DOJ Accredited Representatives, Citizenshipworks. When you come in person for your appointment, DOJ Accredited Representatives will access your application through the website and make any necessary changes. If you do not have access to a computer or need help using the computer, please call Monica at 765) 423-2691 

Step 1: 

Go to https://www.citizenshipworks.org/portal/lum and click the green button to start your application now! 

Step 2: 

Once you answer the initial screening questions (age, green card date, etc.) you will be directed to create an account. In order to start your application, you must create an account using your cell phone number or email. Don’t forget your username (cell phone number or email) and password! 

Step 3: 

When you sign in for the first time, you will receive a pop-up notification asking you to join [org name]. This allows our Accredited Representatives to access your application. Click the green option, “Join.” 

Step 4: 

Click on “Application” and begin! Complete as much as you can before your appointment date. If you complete the entire application, you will be able to save time in your in-person appointment. Please make sure that you answer each question truthfully and honestly. After you are finished, please send an email to [org email] that you completed your application. DOJ Accredited Representatives will not be able to assist you if you have not completed 90% of the application, and you will be referred to [our drop-in hours/workshop].

If you received an email confirmation of your application, it has been submitted successfully. If you did NOT receive a confirmation email, your application probably did NOT submit successfully; so, please email, Kristi Hogue at khogue@lumserve.org and let her know.

If you have an ADDITIONAL CHILD that you would like to enroll in the After School Program, please complete another application for an additional child — click HERE.


Next Steps

After School Program application will NOT be accepted until ALL of the below items are submitted.

  1. Fill out the Online Application Form
  2. Pay $110 Monthly Fee per child; or $35 per week
    • Pay Online HERE -or- Pay in Person -or- Pay by Mail
  3. Submit the following for each child:
    • Immunization Record – current year
    • Individualized Education Plan (IEP), if applicable

ID Clinic Assistance Request

Due to COVID-19, the LUM Office is closed for visitors. If you wish to request assistance in obtaining an ID during this time, you must complete the request form below. LUM staff members will review your request each THURSDAY morning and call or email you. For more information about the LUM ID Clinic, click HERE.

Financial Assistance Request

Please Note – This Form is ONLY for Individuals who are NOT able to Request Assistance In-Person (M-F – 8:15AM) due to Disability, Work Schedule or Illness


LUM staff members will review your online request and call or email you between 8:30 and 11:00 a.m. Monday thru Friday after your application and supporting documentation (lease or utility bill, etc) is received. After School Program Payment Expectations

LUM After School Program is open based on the Lafayette School Corporation Calendar.  

LUM After School Program is open based on the Lafayette School Corporation Calendar.  The After School Program has a set tuition per child, which takes into account all breaks and holidays. Fees are charged regardless of dates/times services are used, as we base our fees on services provided over the course of the semester.  We ask that you commit to paying weekly, biweekly, or monthly. Please understand that timely payment is vital for the functioning of the program.  Our fee covers the cost of providing a safe and nurturing environment, tutoring/homework help, daily snacks, educational activities, field trips, Purdue-sponsored events, and recreational activities.  At this time, we plan to be closed for all school holidays and breaks.  Please look at the LSC calendar.

Monthly Tuition Option: Due, in full by the first of each month. 

Weekly (or Biweekly) Tuition Option: Weekly or Biweekly Payments should be made in advance on the Friday before the covered period.  For Weekly or Biweekly options, the listed late fee will be charged after Monday pick up on the week(s) covered.  

Late Payments:

Payment is expected, in full, by the due dates listed based on your choice of payment option. A $10 late fee will be applied to payments made after the due date. If payments are not made on the due date, and no payment arrangements have been made, services will be suspended until payment has been made in full.  There will also be a late pick up fee assessed at $5 for 15 minutes or any part thereof after 6 p.m.  Continued late pick up will result in loss of services.

CCDF Funding:  Your swiping of hours and payment of weekly co-pay (if applicable) are required forms of payment for you.  If you fail to pay or swipe hours on time, you may be in jeopardy of losing your care at LUM.  Please keep up to date with those expectations.
