Help Getting an ID – COVID-19 Response below

  • Help in getting a valid ID card (Indiana identification) and/or birth certificate

How to Use the ID Clinic:

  • COVID-19 RESPONSE
    • Due to COVID-19, the LUM Office is closed to visitors. Here is how you may request help in getting an ID
      • Complete online ID Clinic Request form, click HERE
      • Submit all documentation through email to gsp@lumserve.org
      • AFTER all documentation has been submitted, LUM staff will email or call you to follow up on THURSDAY mornings only
    • PLEASE NOTE – Due to the high number of requests for ID Clinic assistance that LUM is receiving at this time, you may not hear back from us immediately after submitting the “request form.” One of our staff members will get back to you via email or phone as soon as they are able to get to your request. In the meantime, please submit the request form and then send all documentation (see below) related to your request to gsp@lumserve.org. For more information, please READ this entire page. Thank you for your patience.
  • The service is free, but the entire process may take a few weeks to complete

Who may Use the ID Clinic:

  • Tippecanoe County residents

What you should bring with you:

  • Information – names of birth parents, city & state of where you were born, name of schools you attended and years of enrollment
  • Birth certificate, if available
  • Social Security card, if available
