Salvation Army – Christmas Assistance Program

- Monday, November 6 – Friday, November 10, 2023;
- 9 a.m. – 3 p.m.
- What to Bring: Photo ID, Household Income, SS cards & proof of birthdate for all children

If you wish to participate in Jubilee Christmas, you must complete the request form below. LUM staff members will review your registration form ONLY if ALL of the information requested is submitted and you have verified your income.
Please note, your Jubilee Christmas registration is not final until it is reviewed and accepted by LUM. Registration will be review only if all of the information is included. In order to be considered for Jubilee Christmas you must complete the following steps:
- Step 1 — Fill out the registration form below
- Step 2 — Click SUBMIT.
- IMPORTANT — Stay on this page until the form is done processing. You will receive an email confirmation, if it is submitted properly.
- Step 3 —Verify income eligibility, see below.
Your request will NOT BE REVIEWED until the registration form has been received by LUM staff and and your household income has been verified.
