How to Sign Up & Get Started:
- Call Lafayette Urban Ministry at 765-423-2691
- Make an Appointment — one week window when your return will be done
- Provide contact info and best times to call you
- First come, first served — limited number of appointments
- If you sign up and change your mind — PLEASE call LUM and cancel your appointment so another family may use it
- NO walk-ins
How to Send your Tax Information (AFTER you have scheduled an appointment by phone):
- Fill out a questionnaire/upload documents online, click HERE
- Get intake forms in the mail or by email
— Fill out plus a list of all required documents, and
— Make a no contact Saturday appointment
— Come in and get everything scanned
During your Appointment Week
- Two Phone Calls — one for intake interview and one for final review. You must answer your phone.
- Final Review—Before your final review, you will be sent a copy of your return by secure email

Last Steps
- After final review, come to the LUM Office (420 N 4th Street, Lafayette, IN) during set daytime/evening hours to sign forms so they can e-filed

Who may sign up:
- Tippecanoe County (Indiana) residents only
- Household combined income of less than $57,000
What you should bring with you:
- Valid Photo ID
- Social security cards or ITIN cards for all persons on your tax return
- Earned Income Information — Forms & information on any income you earned last year (W-2 forms, 1099s, 1099 SSA and/or unemployment statements)
- Interest & dividend statements
- Property taxes receipts or rent paid information (landlord name & address)
- Childcare receipts (provider’s name, address & TIN)
- Education expense receipts and 1098 form
- Health care coverage forms (1095-A, 1095-B 1095-C)
- Health Savings Account information, if applicable
- Health Insurance Marketplace Statement, if applicable
- IRS Notices — Any notices sent to you by the IRS
- A copy of your LAST YEAR’s tax return, if you have one
- Bank routing number & account number — for direct deposit
