How to sign up:

  • Dates: Saturday, February 2 through Thursday, April 4, 2019
  • Times:
    • Saturday 10 a.m. – 2 p.m. – February 2 to March 9
    • Tuesday 10 a.m. – 1 p.m. – February 5 to April 2
    • Wednesday 4:30 – 7:30 p.m. – February 6 to April 3
    • Thursday 10 a.m. – 1 p.m. – February 7 to April 4
    • Last appointment day will be on Thursday 4/4
  • Where: LUM Office, 420 N 4th Street, Lafayette
  • Make an appointment
    • by calling 765-423-2691 or
    • stopping in the LUM Office, 420 N 4th Street, Lafayette
      — Monday-Friday from 8:15 a.m. – 4:30 p.m.
  • First come, first served — limited number of appointments
  • If you sign up and change your mind — PLEASE call LUM and cancel your appointment so another family may use it
  • NO walk-ins

Who may sign up:

  • Tippecanoe County residents only
  • Household combined income of less than $54,000

What you should bring with you:

  • Valid Photo ID
  • Social security cards or ITIN cards for all persons on your tax return
  • Earned Income Information — Forms & information on any income you earned last year (W-2 forms, 1099s, 1099 SSA and/or unemployment statements)
  • Interest & dividend statements
  • Property taxes receipts or rent paid information (landlord name & address)
  • Childcare receipts (provider’s name, address & TIN)
  • Education expense receipts and 1098 form
  • Health care coverage forms (1095-A, 1095-B 1095-C)
  • Health Savings Account information, if applicable
  • Health Insurance Marketplace Statement, if applicable
  • IRS Notices — Any notices sent to you by the IRS
  • A copy of your LAST YEAR’s tax return, if you have one
  • Bank routing number & account number — for direct deposit
