5th Quarter Summer Learning Program

Dates/times — May 29 – August 3, 2018; Monday-Friday, 6 a.m.-6 p.m. (subject to change due to snow days)
Location — Central Presbyterian Church, 31 N 7th St, Lafayette, IN

Important Dates

  • Open House
    • Date/time — Thursday, May 25, 2018 at 6 p.m.
    • Location — First Christian Church, 329 N. 6th Street, Lafayette, IN 47901
    • Please come see the facility, meet staff and get answers to any questions that you may have
  • Deposit fees due — at Open House
    • All deposit fees must be paid in full in order to attend
  • FIRST DAY of 5th Quarter — Thursday, May 29, 2018
  • Memorial Day — Monday, May 28, 2018 — 5th Quarter is closed
  • Independence Day — Wednesday, July 4, 2018 — 5th Quarter is closed
  • LAST DAY of 5th Quarter — Friday, August 3, 2018

Fees & Payment Policies

  • $80 weekly fee per child.
  • If your child(ren) will attend 5th Quarter the whole summer (May 29- Aug 3):
    • $80 non-refundable deposit per child required.
    • The deposit will be applied to the cost of the final week of the program.
  • Weekly fees are due in advance (Friday before the following week)
  • Openings are first come, first serve. Deposit fees will guarantee a spot for your child(ren) for the weeks indicated.

    • Pay when you drop off or pick up your child(ren)
    • Stop by the LUM Office (420 N 4th Street, Lafayette, IN 47901)
      Monday-Friday: 8:15 a.m. – 4:30 p.m.
  • Pay by Mail – Make checks payable to Lafayette Urban Ministry;
    and mail to Lafayette Urban Ministry, 420 N 4th Street, Lafayette, IN 47901

Shot (Immunization) Record

A current shot record with the current year’s date is required before your child(ren) begin(s) the program.

  • Learn in detail what your child will experience in 5th Quarter

How to sign-up your children for 5th Quarter:

  • Submit an application — to START the process, click HERE
  • Fees
    • Pay $80 application fee (non-refundable)
      • Due at the registration
      • Application fee will be used for the last week of 5th Quarter
    • Pay Online HERE -or- Pay in Person -or- Pay by Mail 
    • Fee: $80 per week per child.

Who’s eligible for 5th Quarter:

  • Elementary aged children – pre-kindergarten through 6th grade
  • Limited to 40 children

What you need to submit with your application and registration:

  • Children’s names, date of birth, grade, school name
  • Medical information – medication and allergy information; Immunization Record; Physical Exam form
  • Educational needs information; Individualized Education Plan (IEP), if applicable
  • Emergency contact information
  • Read and agree to the Discipline/Guidance Policy
